With electronic transmission, do dentists need to have an authorization form signed by new patients?

Yes. All patients must sign an authorization form so that electronic claims can be sent to insurers. This form is signed just once and is kept in the patients’ files. The declaration, which is found in the Réseau ACDQ Introductory Handbook for Dental Offices(French only), reads as follows:

I hereby authorize the electronic transmission of all information contained in my dental insurance claims to my plan administrator and my plan administrator’s authorized representatives. This authorization will remain in effect until I revoke it.
