Should dentists sign the confidential medical-dental questionnaire?
Dentists do not have to sign it. This form is a unilateral declaration by patients, who enter information about their health condition on it. Therefore, it is the patients, or their legal representatives, who must sign it.
As for dentists, they are required to read the questionnaire. They must also inquire about all changes at every visit, and submit the questionnaire periodically to patients for a full review. Nevertheless, there is no reason why dentists cannot sign it if they so desire.
