Forms

To help you ensure that everything runs smoothly in your dental office, the ACDQ has produced a variety of forms, available at a low price.

ELIGIBILITY

Members in good standing of ACDQ for all forms, non-members for some of them.

PROCEDURE

Non-members may not use the online store and must obtain an order form from Diane Gauthier, ext. 264, or Marie France Auger, ext. 262.

If you order forms through the online store, you must pay by credit card.
However, you can also order by mail, telephone or fax. Payment in full must be received before forms are shipped. It can be made either by credit card or cheque made payable to the Association des chirurgiens dentistes du Québec. On your order, you must specify the name and the quantity of the forms you want, as well as language desired, where applicable.
